When you arrive for your first visit, please be prepared to complete our insurance and health information forms that will allow us to begin your dental treatment.

Our New Patient Forms are also available online so that you can complete them at your convenience, giving you time to carefully review our policies and procedures without having to do so at our office.

If your first visit is for a dental emergency, only the tooth in question will be addressed. Depending on the diagnosis, an additional appointment with adequate time may be necessary.
